Initial Visit with previous assessment completed by another provider

This appointment is for clients who have already been assessed by an SLP and have an assessment report identifying areas of need. This appointment includes: - Time to review previous report(s) - one hour of direct intervention time/ additional assessment to make a plan for treatment - Written summary of results The appointment will include trial therapy and discussion of a treatment plan.
Early Language Assessment
$375
This is an assessment for children ages 0 -35 months who are not yet talking or are not using as many sounds or words as we would expect or for older children who are non-verbal. We assess your child's play skills, pre-linguistic skills, as well as receptive and expressive language skills. Billed time is broken down as follows: - 1.5 hours of direct assessment time - Time to score results and write a summary report with recommendations - A follow-up phone call to discuss results and treatment plan The assessment will include: - Case History - Observation of your child during play - Parent interview - Completion of a standardized checklist to measure expressive and receptive language skills *Please note: an additional fee will be added if a formal report is required for an application for Program Unit Funding (PUF) or an assessment at the Infant and Preschool Assessment Service (IPAS) at the Glenrose
Single Speech Sound Assessment
$225
This assessment is for children who are only having difficulty with one sound such as the R sound or a lisp. This includes: - 1 hour of direct assessment time. - 30 Minutes to write a summary of the results *If myofunctional challenges are suspected to be impacting speech, further assessment may be recommended at an additional cost. The assessment may include a conversational speech sample, oral mechanism exam, and direct assessment of the sound identified as challenging, and discussion of a treatment plan and goals.
Speech Sound Assessment / Informal Language Assessment
$375- $525
This assessment is for concerns regarding pronunciation of sounds (more than one sound) and/or language concerns (e.g., difficulty forming sentences, using correct grammar, following directions, etc). The assessment includes 2.5 - 3.5 hours of billing time depending on the complexity of the speech/language disorder. The time is broken down as follows: - 1 to 1.5 hours of direct assessment - 1.5 to 2 hours to score and analyze results which includes reviewing recordings of speech samples taken during the assessment and write a summary report - A follow-up phone call to discuss results The assessment will include: - Case History - Gathering of a speech sample during play/conversation, completion of a speech sound assessment - Completion of an oral mechanism exam to rule out any structural areas that may need to be addressed - If there are language concerns, informal language assessments will be completed to determine specific areas for treatment. *Note: While informal language assessments are very useful to guide and plan treatment, they cannot be used to qualify for funding
Formal Speech and Language Assessment for Funding Purposes

This assessment is for children with multiple areas of concerns (e.g., pronunciation, forming sentences, and following directions). This assessment may be completed over 1-3 hours (which may be broken into 2 appointments). This assessment includes: - Case history - Parent Interview - Consultation with school team if appropriate - Speech sound and oral mechanism evaluation - Standardized, norm-referenced assessment of spoken language (understanding language and using language) - Phone call to discuss the results - Written report with speech-language diagnosis, recommendations, and strategies - Support to find an appropriate community placement or pre-kindergarten program
Orofacial Myofunctional Assessment
$400
This assessment is for children with concerns regarding the muscles of the facial structures including. Some examples of these concerns may be an open mouth resting posture, mouth breathing, drooling, or a tongue thrust. These issues may be impacting speech and preventing your child from generalizing sounds into everyday conversation despite extensive speech therapy. They may also be causing your child's speech to sound "slushy" or "lazy" in connected speech. Billed time is as follows: - 1.5 hours of direct assessment time - Time to analyze results and write a report - Phone call to discuss results This assessment includes: - Case history - Parent interview - Speech sound assessment - Extensive Oral mechanism exam - Swallowing assessment - Phone call to discuss the results - Written report with speech-language diagnosis, recommendations, and strategies - Referrals to other professionals as needed

5-10 minutes for completion of the session note. This may be completed while the child has free play at the end, or the session may end 5-10 minutes early to leave time for documentation.
